Informed consent

For all experiments, fMRI and psychophysics, you need to have the informed consent from subject. First, you need to explain about subject rights and possible matters described in the experiment explanation. NOTE THAT EVERYTHING WRITTEN IN THE FORM SHOULD BE FULLY UNDERSTOOD BY SUBJECT. The signed consent form must be kept in place where it is locked.

Psychophysics

Our informed consent forms and the explanation are stored in the drawer of the desk. You will explain about it, ask subject to read the form. If the subject agrees with what is written, ask him/her to fill the columns for their consent. Once it's done, put the signed form into the cabinet behind the chair and lock it. The key is attached to the key holder with the door key.

fMRI

There are two inform consent forms, one is ours, the other is fMRI support unit one. Experimenter could find our consent form in the drawer at fMRI room(the right one in the picture).

The support unit form can be printed out from the subject database in their computer(left one on the desk). If you're not sure, please ask support unit staffs. You'll make sure that the subject understands fully for both explanations and have his/her signature on the support unit form. And we need subject to fill our consent form too. Once you have both done, you can ask subject to be ready for fMRI session.
After the experiment, ask subject to fill the support unit questionnaire. Make sure first 5 items are not marked “Yes”. If subject report any abnormality in these five items, take care the subject and ask the detail. It must be reported to the support unit. The filled forms and questionnaire will be taken back to our lab. Have a copy of both support unit consent form and questionnaire then put these copies in the folder at the cabinet in the support unit office, not fMRI room.

You can find them in our repository, gru. Please note that the file names are Japanese. For example the English consent form is “実験同意書(英語).doc”. I put links on the desktop of each computers to the repository.

List of psychophysics subjects

This is to coordinate signing of consent forms. Particularly helpful for sending a bunch of forms to people who are currently overseas. But also for asking local people only once and then giving them forms in one go, if possible, rather than in multiple sessions.
